Passport requirements: Original passport valid for at least six months, signed in designated place with at least 1 blank page

One (1) completed online application form: Print receipt and sign inside the appropriate box in blue or black ink only

Two (2) recent colour photo (scanned photos not acceptable) Photo should be 2″ x 2″ with a white or light coloured background, neutral expression with mouth closed and no hats or sunglasses .

Photocopy of round trip ticket to Brazil, OR a copy of itinerary on a Travel Agents company letterhead.

Police clearance certificate: Issued by the RCMP or local police within the last 6 months

Letter of enrollment: Issued by school, college or university in Brazil confirming enrollment or acceptance of the student in the exchange program.

Registration letter: From school, college or university in Brazil.

Letter of Brazilian sponsor: Issued by Brazilian exchange students program organization stating the student’s inclusion in the program, name and location of the educational institution as well as the address/names/qualifications of “hosting family” in Brazil.

Proof of financial capability to pay for stay in Brazil.

Address for students stay in Brazil.

Service request form (SOF):This is EmbassyLink’s Service Order Form. You must include a copy of this form with all other supporting documents.
Notes:
Travellers under the age of 18 will require:

A letter from both parents authorizing the minor to apply for a Brazilian visa

2 x original notarized authorization forms signed by both parents

2 x photos

Copy of birth certificate with parents’ names

Copy of parents identity (passport or id card)

In the case of divorced parents – a copy of the judgement granting guardianship
