Tourist visa requirements

• Passport requirements: Original passport valid for at least 2 months past intended date of departure, signed in designated place with at least 1 blank page.
• One (1) completed and signed application form: Original signature in blue or black ink only.
• One (1) recent colour photo : (scanned photos not acceptable) Photo should be 2″ x 2″ with a white or light coloured background, neutral expression with mouth closed and no hats or sunglasses.
• Service request form (SOF): This is EmbassyLink’s Service Order Form. You must include a copy of this form with all other supporting
